Gentiva is there for those in need. Assisting others is the foundation of what we do each and every day. Caring, supporting and striving for the best in ourselves and each other is paramount in the home healthcare industry, and are just a few of the qualities that make this world a better
place for us all.

In that spirit, we have the opportunity to help Gentiva coworkers and their family members in need through the John T. Ennis, Sr. Foundation, created in memory of Corporate Counsel and friend John T. Ennis, Sr., who passed away in 2005.
John T. Ennis, Sr. was a counselor in many ways. In his professional life he served the Gentiva family of companies for over 20 years. In his personal life, he acted as a mentor and friend to countless individuals who needed advice, guidance or assistance. His generosity of spirit and endless quest to help those in need profoundly touched his family, friends and acquaintances. Now, with contributions to the John T. Ennis, Sr. Foundation, we can continue his legacy and make a difference in the lives of others, as he did.
The John T. Ennis, Sr. Foundation is a 501 (c) (3) nonprofit corporation whose mission is to help people who have chosen to help others through homecare. Gentiva supports this foundation through its own donations, and it’s available to assist our associates and their family members in times of catastrophic or devastating events such as hurricanes, death, severe loss or other extreme tragic circumstances.
In instances of national disasters, the John T. Ennis, Sr. Foundation may also make donations to organizations like the Red Cross with the approval of the Board of Directors.
There are three ways to authorize a donation:
- Have a donation deducted from your payroll each pay period (Donations can be as little as $1.00 per pay period!)
- Have a donation deducted from your payroll for one pay period, or over a designated amount of time
- Make a one time cash donation
Each donation made by associates will be matched by Gentiva, up to a total company match of $100,000. Click here for the donation form. To make a single donation, please make your check payable to The John T. Ennis, Sr. Foundation and mail to: Lori Ennis, John T. Ennis, Sr. Foundation, 3350 Riverwood Pkwy. SE Suite 1400, Atlanta GA 30339. All donations are tax deductible.
To be eligible for assistance, an associate (or
immediate family member) must be faced with untimely, catastrophic or devastating events such as hurricanes, death, severe loss or other extreme tragic circumstances. There are many tragedies that qualify for assistance. There are also many that do not qualify based on the IRS’s standards. Click here to learn more about purpose and eligibility.
