“Your generosity will stay with me long after the financial assistance is gone.”
I am a single parent of two who was diagnosed in March, 08 with stage four colon cancer that metastasized to my liver and pancreas. I was the picture of health before this diagnosis – aside from a cold now and then –- so this was a huge shock. I have experienced a decrease in income since I’m unable to work, and an increase in medical bills. A grant from the John T. Ennis, Sr. Foundation helped me to get back on my feet and gave me invaluable peace of mind.
I’d like to think that someone reading this will realize the importance of the Foundation. I didn’t realize how amazing it was until I needed it myself. I am so thankful to work for a company that truly cares about its associates. I extend my sincere thanks to the Foundation and to my fellow coworkers for impacting my life in such a positive way and on an emotional level that has stirred my soul. Your generosity will stay with me long after the financial assistance is gone.

“I never thought that I would be in a position to require anyone else’s help, but when my health failed me the John T. Ennis Sr. Foundation was there.”
In March of 2008 I went in for a hysterectomy. Months before the surgery I saved money so I’d be able to pay my bills during this time. When I arrived at the hospital, I found out I was required to pay $1000 down before the surgery could be performed, which wiped out my savings.
To top it off, I had complications from the surgery which required me to stay in the hospital for another 6 days, and I ended up having another surgery to remove my parathyroid glands.
During this time I had no money coming in and was totally reliant on the help of other until I received the grant from the John T. Ennis, Sr. Foundation.
With your assistance, I was able to pay some bills and buy groceries. This really helped to get my self esteem back. I shall be singing the foundation’s praise and donating to this cause for the rest of my life.
Thanks so much,

“A company can be a company, but it truly takes loving, caring adults to create such
an exceptional business.”
I’m writing in response to the contribution the John T. Ennis Sr. Foundation has made on my life and how wonderful this foundation is. First and foremost, I want to thank the foundation and my colleagues in the Danville / Lincoln, KY office. It’s hard for me to even consider saying office because they have been such a wonderful family to me. I have never experienced such comfort and care from any other company I have worked for. A company can be a company, but it truly takes loving, caring adults to create such an exceptional business. I’m so very proud to be part of the Gentiva family.
In October or 07, my Ocular Histoplasmosis became active and I began undergoing treatment to stop the hemorrhage. With assistance from the John T. Ennis, Sr. Foundation, I was able to pay for outstanding medical bills. I am still undergoing treatments and trying to deal with the deficit in my vision.
We are never prepared for things like this, but with the help of my friends at Gentiva, I’ve been given hope and now have a more positive outlook on my future. This has only made me a stronger person with a heart full of gratitude.

“We very much appreciate your kindness and willingness to reach out to us in our time of need.”
I want to express our gratitude regarding the generous contribution our family received from the John T. Ennis, Sr. Foundation.
Our daughter, Kaitlyn Marie Sudberry was brutally and tragically taken from us on January 28, 2008. She was 17-years-old and had just started her last semester of high school. She had been accepted to Northern Arizona University and was planning to go into Wildlife Science. She had a very kind and loving spirit and will be forever loved, remembered and missed.
When a family is faced with such an unexpected event, the effect is overwhelming. Kaitlyn has two sisters (Yvonne, 24 and Virginia, 14) and 2 brothers (RJ, 22 and Dan, 20). RJ is in the Air Force and is stationed in Ft Walton Beach, Florida and Dan was residing in Chicago, IL, at the time of Kaitlyn's passing. Yvonne lives close by and Virginia lives with us. After being told of Kaitlyn's passing and moving past the hysterics, all we could think of was gathering our children, near and far. This meant getting airline tickets at a moment’s notice for our sons.
The donation from John T. Ennis, Sr. Foundation helped curve the monetary stress we were under. We very much appreciate your kindness and willingness to reach out to us in our time of need.

“I am now giving back to help someone else.”

I had planned on an uneventful partial left knee replacement on July 26, 2006. While I was still receiving blood thinners, I sustained a large bruise on my left breast. Very concerned, I requested a diagnostic mammogram and ultrasound. The doctors told me that “yes there was a bruise on the left but there was also a lesion on the right”. I had a lumpectomy in September and then a re-incision in October as the lesion was larger than the estimated size of the biopsy.
I was also hospitalized in November for a DVT and Pulmonary Embolism, which delayed starting my chemotherapy treatments. Chemotherapy was then further delayed, as I was to pay up front for services since I had not met my insurance deductible. I am so thankful to The John T. Ennis, Sr. Foundation. Your generous gift helped me to start treatment, and I am now a Breast Cancer survivor.
To surprise me, my oldest grandson Brett, made a donation on my behalf and had a pink hair extension glued into his hair as a Breast Cancer Awareness campaign at school.
I am now giving back to help someone else. I’ve included pictures of me with my grandchildren, Brett, Elena and Matthew.

“Thank you all for donating to this fund. Your generosity truly helped me.”
I was recently diagnosed with a chronic condition that caused me to be out of work for eight weeks. I did not qualify for Short Term Disability, and my PTO benefits didn’t cover all of my expenses including medical bills and prescription medications. I didn’t know how I was going to survive.
When I received the letter from the John T. Ennis, Sr. Foundation saying I was a recipient, my first response was to cry tears of happiness. I have been going through a lot and it was truly a blessing to receive help when I did. Thank you all for donating to this fund. Your generosity truly helped me.

November of 2005 through November of 2006 was one of the toughest times that my wife Deanna and I have had to struggle through. Last year, Deanna was diagnosed with breast cancer and the series of events that followed related to her illness have caused us to - as my father used to say -"Do the best with what you have been dealt." With Deanna's surgeries, chemotherapy and radiation treatments, along with her inability to work at that time, we saw our emergency fund wither away to almost nothing. When I was at the end of my emotional rope, I went to see my branch administrator so I could just blow off some steam. When I shared my story, he contacted the John T. Ennis, Sr. Foundation on our behalf. A couple of weeks later a check arrived. Deanna and I do not know what we would have done without such a foundation that seeks to help associates, which is why I now donate a percentage of my pay to the John T. Ennis, Sr. foundation each month. I encourage all associates to get on board with this noble and helpful endeavor. One day you might need help, and the foundation will be there.
Thank you,

“We are
truly thankful to this wonderful
foundation for all of its
assistance.”
In February of 2006, my husband
suffered a broken neck. Following two cervical fusions, he was
transferred to Spaulding Rehab in Boston where he remained until
November. At discharge from Rehab, our home required renovations
including a ramp for his wheelchair. With help from the John T Ennis
Sr. Foundation, we were able to install the ramp. We are truly
thankful to this wonderful foundation for all of its assistance.
